Have your say on the Personalised Care Toolkit for maternity
Women and families across the North East and North Cumbria are being asked to do a short survey. It is about the Personalised Care Toolkit for maternity care.
You might have used the videos and leaflets in the toolkit without knowing. It includes:
• video tours of maternity wards
• information leaflets about choices
• decision-making tools
• videos and leaflets on BadgerNotes
The survey is for people who are pregnant or who gave birth after the toolkit launched in May 2025.

The survey takes around 5-10 minutes to complete and responses are anonymous.
The survey is available in multi languages, including Romanian, Polish, Malayalam and Kurdish -Sorani. To change the language setting, use the drop down box in the top right corner of the survey form, as shown here:
