WEST NORTH EAST CUMBRIA MATERNITY VOICES PHOTOGRAPHY RELEASE FORM

I hereby authorize West North East Cumbria Maternity Voices (further referred to as ‘The Group’) to publish photographs for use in The Group’s print, online and video-based materials, as well as other publications.

 

I hereby release West North East Cumbria Maternity Voices, its volunteers and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation.

 

I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated in the taking or publication of these photographs or participation in group marketing materials or other publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.

 

I understand that I also have the right to access my personal data (photos) upon request and to receive confirmation on how these are being used, in accordance with Article 15 (“Right to Access”) of the General Data Protection Regulation (2018).

 

In accordance with the Article 17 (“Right to Erasure”) of General Data Protection Regulation (2018), I understand that I have the right to request my photos to be removed from any websites or social media. I also understand that I have the right to request my photos to be removed from any future printed materials.
